What are Ambulatory Surgery Centers?
An ambulatory surgery center (ASC) is a medical facility focused on providing same-day outpatient surgical care, including diagnostic and preventive procedures. ASCs aim to perform surgical, diagnostic, and certain medical procedures more cost-effectively than traditional hospital-based outpatient departments. Unlike hospitals, ASCs usually specialize in a specific group of outpatient surgeries such as colonoscopy, cataract removal, orthopedic procedures etc.

Growth of ASC Industry
The U.S.Ambulatory Surgery Center has seen tremendous growth over the past few decades. In 2020, there were over 5,700 Medicare-certified ASCs across the country performing outpatient surgeries. The number of ASCs has doubled since 2000. The main factors that have contributed to this growth are:

Cost Savings: On an average, ASCs can perform surgeries at roughly 60% of the costs of hospital outpatient departments. The lower overhead costs allow ASCs to offer more affordable prices.

Shift to Outpatient Care: More procedures that previously required overnight hospital stays are now being safely performed on an outpatient basis in ASCs. Close to 70% of all surgeries performed in the US today are on an outpatient basis.

Emphasis on Quality: Studies show that ASCs achieve equivalent or better clinical outcomes compared to hospital outpatient departments for many low-risk surgeries despite their lower costs. Their specialized focus on select procedures leads to better quality.

Increased Acceptance: Earlier there were concerns about safety of ASCs but now they are widely accepted even for more complex low-risk procedures. With medical technology advances, more surgeries can shift to the lower cost outpatient setting.

Convenience for Patients: ASCs offer convenient locations, easy parking, individual attention from staff and quick recovery times. They relieve pressure on overburdened hospital emergency rooms.

Revenue for Physicians: Many physicians have an ownership stake in ASCs allowing them to supplement their hospital-based practice income. This encourages more doctors to perform surgeries in the ASC setting.

Profile of a Typical ASC
A typical ASC in the US is a free-standing facility not located within a hospital building. It has 3-8 operating/procedure rooms and focuses on one or two clinical specialties like ophthalmology, orthopedics, pain management etc. ASC owners are usually physicians in partnership with health care investors and real estate developers.

ASCs employ a medical staff consisting of physician specialists, registered nurses, surgical technologists and other licensed professionals who handle pre-op and post-op care of patients. Advanced medical equipment like digital X-ray, MRI and endoscopic systems support complex minimally-invasive procedures. Strict guidelines are followed around patient safety, sterilization procedures and handling of medical waste.

The Business of ASCs
According to industry analysts, the US ASC market was valued at around $40 billion in 2020 and is expected grow to over $60 billion by 2027. The top ASC management companies in the country include Surgery Partners, United Surgical Partners International and Surgical Care Affiliates.

For physicians, ASC ownership offers an average annual return on investment between 13-18% compared to roughly 5-8% for hospital-based practice. This makes ASC a lucrative long-term investment. However, the development and maintenance costs are high as ASCs must consistently upgrade facilities and technologies to remain competitive.

Strict state and federal regulations around ASC construction, credentialing, accreditation and complex compliance add to operational challenges. Maintaining high patient volume is critical for financial viability as fixed overhead costs are to be covered. Overall, ASCs compete intensely to offer value by balancing clinical excellence, patient satisfaction and cost-efficiency.

Future Prospects
With healthcare moving towards more preventive, chronic care management models, ASCs are well-positioned to take on additional low-risk, non-surgical procedures in future. The development of imaging and diagnostic technologies could allow more procedures to shift from hospitals. With dedicated physician leadership and focus on outcomes, ASCs are likely to play a pivotal role in the evolution of outpatient surgery industry across America.
